Weight Loss Program
The primary goal of our Weight Loss Program is to help you reach a healthy weight, but it doesn’t end there. Our program is customized to the needs of each patient, taking into account body composition, diet, fitness routine, and behavioral patterns.
There are many factors that can keep you from losing weight at any age. By identifying those barriers, incorporating medications to aid in weight loss, and tracking your progress along the way, our program will help you break through plateaus and get the accountability and motivation you need to reach your goals.

The Weight Loss Program is a self-pay program. We do not file insurance claims on weight management. We can accept cash, credit, or debit from checking or health savings account.
Initial Office Visit - $150.00
Determining which program level and medication needs will help you achieve the best results, will then determine your monthly cost. Those options include:
Level One - $75.00 / month
Level Two - $350.00 / month
Level Three - $500.00 / month

The initial visit includes:
Medical and Family History Review
Cardiovascular evaluation
BMI assessment with recommendations
Weight loss goal establishment
EKG or lab work, if indicated

All patients on the Weight Loss Program must adhere to the following schedule to ensure proper treatment and prescription refills. Prescriptions are dispensed in 4-week quantities from a local or mail order pharmacy. Monthly health evaluation and weigh in will be required for refills.

You may book an appointment online or over the phone, noting your interest in the weight loss program and your initial visit.
Chronic Care Management Program
Our Chronic Care Management Program was developed to provide an additional level of care and support for patients managing two or more chronic conditions. Program participants are assigned a dedicated Care Coordinator Nurse who assists with medication management, scheduling labs and other testing, coordinating visits with other providers, and developing a custom comprehensive care plan.
Our goal is to ensure patients receive the best care possible across specialists, stay out of the hospital, and minimize costs and inconveniences due to unnecessary visits to the doctor.

Medicare will allow us to bill for these services during any month that we have provided at least 20 minutes of non-face-to-face care of you and your condition.

As needed and as part of coordinating your care, we will share your health information electronically with other providers involved in your care in compliance with HIPPA.

You have the right to discontinue this service at any time for any reason. Because your signature is required to end your chronic care management services, please notify us if you would like to stop services.

Patients with two or more chronic conditions may sign up in-person at our office. We’ll review all aspects of the program, discuss your current care plan, and provide consent forms.
